Communication and Cancellation Policy

Payment Information

Payment for services can be made by cash, credit card, HSA/FSA cards or check made out to Gabriel Katz MD.  Payment is expected at the time of each appointment. I will provide you with a detailed receipt to submit to your insurance company at your session.

Cancellation Policy

There is a 48-hour cancellation policy. Fee for appointments cancelled less than 48 hours prior result in a charge of 100% of the usual office fee.

Length of Sessions

An initial session ranges from 60-90 minutes. Follow-up psychotherapy sessions are 50 minutes.  Psychopharmacology sessions are 30-45 minutes depending on a person’s need.

Contact

My preferred method of contact is by telephone. While I do accept medication refill requests via email/text, good medical practice may mean that is necessary for me to speak with you or been seen in the office prior to a medication refill.

New patients should call and leave a voicemail.

Privacy Practices/HIPPA

Your private health information is completely confidential unless you give me a written consent to discuss your information with someone you and I designate for the purposes of your care. An exception to this rule may be made if I believe either you, or someone else is in danger.

Do you accept any insurance plans?

I do not accept insurance. I will provide a receipt for insurance at each session, if needed.
